If you would like to join the PTA and/or have your children’s information included in the school directory, please return this form to the PTA mailbox by Wednesday September 27. Any forms not received by that date will not be included in the directory! Even if your information was in last year’s directory, we need your approval again to publish updated info.

(Please complete both sections of the form as they are for two separate things…we’re just trying to conserve paper.)

 

Of course, you may join the PTA at any time during the year. You don’t need to be able to attend the meetings to join! If you are not able to attend meetings and would like the paperwork from the meetings sent home to you, please let a member of the Executive Committee (listed on the back of the school folders) know. If including dues payment, please either staple it to the form or put them together in an envelope.
