Seniors planning to apply to college must print and fill-out the following required forms. These forms are required before any counselor letter of recommendation can be written for any student.
Students should bring these completed forms to their guidance counselor as they begin the college application process.

The Activity Sheet - student completes this form and gives it to their guidance counselor.

The Personality Checklist - student prints out three (3) copies of this form; completes the top portion of each; gives these form to three different teachers. Teachers complete the form and return it to the student's guidance counselor.
