Dear PreK -12 District Staff attending on August 29th:

This serves two purposes, one is to inform the PD Committee for planning purposes.  The second is for you to have a product/documentation for your PDP records.  Once you complete the form, you will automatically be sent a copy of your responses.

Please be sure to sign the attendance sheets in your sessions, in order for the district to record your participation in Vector Solutions/Teachpoint.
